Owner Well Id as referenced on project site map Drill Hole Diameter in inches Casing Diameter in inches Surface Seal Depth in feet Maximum Depth in feet Latitude and Longitude (optional)
for Well Destruction you will also need:
Original Permit # issued for well construction (optional) State Well # assigned to well constructed (optional) DWR # for the well constructed (optional)
