You will be asked to provide the following information about your wells:

• Owner Well Id as referenced on project site map
• Drill Hole Diameter in inches
• Casing Diameter in inches
• Surface Seal Depth in feet
• Maximum Depth in feet
• Latitude and Longitude (optional)

For Well Destruction you will also need:

• Original Permit # issued for well construction (optional)
• State Well # assigned to well constructed (optional)
• DWR # for the well constructed (optional)
