The County of Alameda has adopted an electronic identity verification process, which eliminates the need for a notarized authorization statement. If you are unable to pass the process or choose not to use this service, you can follow the instructions for the Notarized Authorization Statement.
After submitting your online order, a pre-filled Authorization Statement will be prepared for each certificate requested that requires it and available for printing from the confirmation page and email.
The statement must be notarized (signed in presence of notary public) and submitted to our office before the order can be processed. If supporting documents are required, they must be submitted with the notarized statement.
NOTE: If you are requesting multiple copies of the same certificate, only one notarized authorization statement is required.

The person who submitted the request must present official government identification and sign the statement under penalty of perjury upon arrival. If supporting documents are required, you must submit those documents with the unsigned authorization statement to our office before the order can be processed. Failure to do so may result in cancellation of your order. Your order will NOT be processed until required documents are received.
For unrestricted copies, if the request is to be delivered via mail or express delivery:An electronic identification verification or a notarized authorization statement is required. Any applicant who fails or opts out of the electronic identify verification will be required to submit a notarized authorization statement before the order can be processed. The statement must be notarized (signed in presence of notary public) and submitted to our office before the order can be processed. If supporting documents are required, they must be submitted with the notarized statement.
Note: International requests for unrestricted copies must be notarized at a United States Embassy.
