Request Proof of County Insurance

If your Department is in need of proof of County’s insurance coverage, please email Sean Mullen, Risk Coordinator. Risk Management will need the following information to process your request:

• Certificate Holder Name
• Event or contract description
• Event date or the start and end date of the contract
• A copy of the event application or copy of the contract insurance requirement
